I have enrolled in a program of strenuous physical activity including but not limited to weight training, stationary bicycling and various aerobic conditioning offered by JMR Fitness LLC dba JMR Online Fitness Specialists. I hereby affirm that I am in sufficient physical condition to undertake such a program, that I have had a medical examination and clearance from my physician to perform under the condition of a personal training session or on my own as a member, that I do not suffer from any disability that would prevent or limit my participation in an exercise program, and that I will notify my trainer and my physician if I should encounter an injury or symptoms of an injury (e.g. discomfort) as soon as such conditions arise. I will strictly follow the instructions as given by my trainer and will abide by the JMR Online Fitness Specialists safety guidelines.
In consideration of my participation in JMR Fitness LLC dba JMR Online Fitness Specialists exercise program, I, for myself, my heirs, and assigns, hereby release, discharge and covenant not to sue JMR Fitness LLC dba JMR Online Fitness Specialists, it’s employees, owners and contractors from or for any claims, demands and causes of action arising from my participation in exercise programs at or provided by JMR Fitness LLC dba JMR Online Fitness Specialists .
I fully understand that I may injure myself as a result of my participation in JMR Fitness LLC dba JMR Online Fitness Specialists exercise program and I hereby release JMR Fitness LLC dba JMR Online Fitness Specialists from any liability now or in the future including, but not limited to heart attacks, muscle strains, pulls or tears, broken bones, shin splints, heart prostration, knee/lower back/foot injuries and any other illness, soreness, injury or death, however caused, occurring as a result of during, or after my participation in exercised programs provided by JMR Fitness LLC dba JMR Online Fitness Specialists.
By checking YES in the space provided at the bottom of this document, I hereby affirm that I have read and fully understand the above, and I UNDERSTAND THAT BY CHECKING YES, I AM WAIVING CERTAIN RIGHTS WHICH I OR MY HEIRS, NEXT OF KIN, EXECUTORS, ADMINISTRATORS AND ASSIGNS MAY HAVE AGAINST JMR FITNESS LLC dba JMR ONLINE FITNESS SPECIALISTS. I ALSO UNDERSTAND THAT I MUST BE AT LEAST 18 YEARS OF AGE AND ABLE TO ENTER A CONTRACTUAL AGREEMENT TO PARTICIPATE IN A JMR FITNESS LLC dba JMR ONLINE FITNESS SPECIALISTS PROGRAM.
